VHG Confirmation of Sessions letter template
1. Purpose
This template is to be used if a client requires confirmation that they attended psychological therapy sessions with VHG
2. Process
To download a copy of the notes template click on the "Attachment" icon at the bottom of this page.
This will then appear above your task bar at the bottom of your computer screen for you to use.
If this does not appear go to “File Explorer” and then into “Downloads”.
3. How to use this template
When you have downloaded the template, you can amend it as necessary with the addressee details and the client details. The fields you will need to change are highlighted in red. Please delete any text in red and fill it in with the appropriate information and the number of sessions attended. Once you have filled in the necessary information, the font colour will need to be changed to black.
This template is designed to provide confirmation that the client attended sessions for the presenting issues they described on referral/initial assessment. Therapists should not provide any opinions about how a client has engaged in sessions, their progress, or any other subjective information in this letter. The letter can be provided to the client on completion for their own use.
If a client requires further support with their mental health, the GP Letter Template should be used to detail the support required and this should be uploaded to MPB to be sent to the client's GP, with the client's consent. The client can also have a copy of this sent to them if they wish.
