VHG Treatment Notes Template
1. Purpose
This form is to be used only during system downtime of the “Making People Better” case management platform.
2. Process
To download a copy of the notes template click on the icon at the bottom the this page.
This will then appear above your task bar at the bottom of your computer screen for you to use.
If this does not appear go to “File Explorer” and then into “Downloads”.
All sections of this form must be completed. If the section is not applicable at the time of the appointment, please enter “Not Applicable”.
Once you have completed the session notes, please submit via https://vitahealthgroup.zohodesk.eu/portal/en/newticket and attached a copy of the treatment notes.
