To provide a full how to guide on discharging a patient on the VHG platform.

Click on the “Patient” tab.

Search for the clients file.

Then navigate to the “Health History” tab.

Click on “Add Reports & Letters” button.

Then at the bottom of the page from the drop down list select the letter template you require.

Once you have selected the required entry fields will appear below. You will need to complete all mandatory sections that are marked with a red Asterix *, some will already be pre-populated where possible. Entry prompts will appear in grey text within the fields.

Once you have completed all the required fields you will have two options at the bottom of the page.

Please note: once you click on “Save Reports & Letters & Complete” you will not be able to go back in an edit.
